The following summary provides a brief overview of our benefit plans.
Medical and Dental Insurance
PGV offers regular full-time employees a comprehensive basic and major medical insurance plan with prescription drug card, and a dental insurance plan. Plan details are reviewed with the employee by Human Resources during the orientation process and through plan summaries and booklets distributed in the benefits package. New employees must apply for coverage within the enrollment period (31 days from date of hire). Enrollment after this period will require a statement of good health to the insurance company who will then determine coverage eligibility based on the employee’s medical history.
Eligible employees may elect dependent coverage for their spouse and unmarried children from birth to age 19, or up to age 24 provided that the child is unmarried and a full-time student. Employees may elect coverage for newly eligible dependents (such as spouse following marriage or child following birth or adoption) within 31 days from the date of eligibility without having to provide medical evidence of insurability.
Regular full-time employees are eligible to enroll when employment begins (date of hire). Medical coverage ends on the first day of the month following separation of employment. Dental coverage ends on the 15th on the month of separation of employment.
Tricare Supplement Insurance
A supplement to military health benefit for employees who are military retired.
Voluntary Life Insurance
The Voluntary Group Life Insurance Plan offers employees and their dependents an excellent opportunity to purchase affordable group term life insurance. The important plan features are high limits, guaranteed acceptance, and conversion and portability rights.
Employees are eligible to enroll on their date of hire.
Medical Reimbursement Flexible Spending Account
The Medical Reimbursement Flexible Spending Account allows regular full-time employees to save money by setting aside pre-tax dollars to pay for eligible, uninsured medical or dental expenses. This account may be used to reimburse the employee and their eligible dependents for out-of-pocket expenses.
Dependent Care Reimbursement Flexible Spending Account
The Dependent Care Reimbursement Flexible Spending Account allows regular full-time employees to submit day-care expenses for eligible children and/or other dependents. The maximum amount of dependent care tax-free reimbursement is $5,000 ($2,500 if the employee and their spouse file separate tax returns.
Private Insurance Reimbursement Flexible Spending Account
The Private Insurance Reimbursement Flexible Spending Account allows regular full-time employees to set aside the cost of premiums with a private insurance company on a pre-tax basis.
Premium Contributions
For regular full-time employees PGV pays a $170.00 Flex Credit per pay period to help pay for medical and dental premiums and/or the other benefits offered (Voluntary Life Insurance and Flexible Spending Accounts).
If the employee elects not to use the Flex Credit for benefits, it may be taken as taxable cash.
Company Paid Benefits
Life Insurance/Accidental Death & Dismemberment (AD&D)
All regular full-time employees will receive Company-paid life ($50,000) and accidental death and dismemberment (AD&D) insurance.
Short Term and Long Term Disability Insurance
All regular full-time employees will receive Company-paid disability insurance. Recognizing that illnesses and injuries may occur, PGV has established a Short Term and Long Term Disability Insurance benefit that provides income continuation protection for employees. Under this program, benefits are payable for non-work related injuries or illnesses.
